AIM: The purpose of this questionnaire is to gather information about the way people working in the poultry industry prefer to receive information about disease control and biosecurity measures; and how that information travels within the industry. The answers you give will be used to improve the future flow of information between the industry, veterinarians, government and other stakeholders, for the benefit of all.

HOW: AHVLA would like as many people as possible from the poultry industry (including managers, farm owners, veterinarians, poultry employees) to complete a short questionnaire. We have produced a survey questionnaire which can be completed either as a paper copy or as an online survey. Complete the questionnaire promptly and return it by 3 February 2012 for a chance to win a £50 voucher. We can also provide hard copies with prepaid envelopes, please email us to receive one.
